Follow-up to #757. When a verification step fails, it currently calls request_recovery and waits for a human. Instead, turn the validator's findings (failing acceptance criteria, check output) into a new fix Issue on the same branch, re-run verification after it, and cap the loop at N rounds before falling back to a human recovery gate.
