poll delivery PR review comments (forge clients already poll merge gates)
policy for required vs optional comments (e.g. only 'request changes' reviews, or a label)
create fix Issues, re-run verification and delivery on the same PR
avoid loops on bot comments

Follow-up to #757. Unresolved review comments on a Factory delivery PR should create fix Issues on the same Factory branch, then refresh the delivery. This is the open question in the old factory-feedback-handling research: who decides a comment is required, where optional feedback waits, and how to react to a merge request mid-run.
