Follow-up to #757. A retried Attempt (after failure, interrupted runtime, or a failed verification) reuses the same model today. Escalate retries to a stronger tier (e.g. implementation model → planning model). Optionally, let the planner tag each Issue low/med/high difficulty and pick the tier from that.
